Nima Fahimian, MD, Medical Director
Dr. Nima Fahimian, is double board-certified in both Integrative Medicine and Addiction Medicine. Dr. Fahimian is well known for advancing the field of psychiatry with his particular expertise in psychopharmacology, biofeedback, and Express TMS. With a head for research and a heart for clinical care, Dr. Fahimian ensures that his patients are treated with the most effective, up-to-date, and safest treatments available. Dr. Fahimian was trained at UCLA David Geffen School of Medicine, where he was appointed to be the Chief Resident during his psychiatry residency. Dr. Fahimian has held faculty appointments at UC Davis, UC Irvine, and is currently an Associate Professor of Clinical Psychiatry at UC Riverside.
Ben Spielberg, M.S. – Founding Neuroscientist & Chief Executive Officer
With almost a decade in the field of brain-based behavioral health under his belt, Ben is more passionate about mental health now than ever. Ben went to graduate school at Columbia University and received a Master of Science degree in Neuroscience and Education. During graduate school, Ben developed a specialty in clinical neuromodulation and neuroimaging, where he used brain scans to predict off-label uses for TMS, neurofeedback, and Ketamine. Ben’s research has been published in prestigious journals such as Brain Stimulation and ACR Open Rheumatology, and presented at The Clinical TMS Society, The International Neuropsychological Society, and the Western Psychological Association. Ben prides himself in bringing together the special team of individuals that makes up TMS & Brain Health.
Meredith Sagan, M.D., MPH – Psychiatrist
Dr. Meredith Sagan is a holistic psychiatrist with a specialization in the treatment of trauma and addiction through mind-body integration. Dr. Sagan has been heavily involved in personal growth since her first trip to India in 1992, where she conducted research on yoga therapy in the treatment for anxiety and established her expertise in stress reduction and life skills education. Since then, Dr. Sagan has found that the common cause of people’s stress, anxiety, and mental health issues stems from a lack of connection with their core self. She has worked with children, adolescents, and adults in foster care systems, correctional facilities, inpatient and outpatient behavioral health settings, and elite residential treatment facilities. Dr. Sagan completed her psychiatry residency at University of California Los Angeles Neuropsychiatric (Semel) Institute and West Los Angeles Veterans Administration Hospital.
Gezel Saheli, M.D. – Psychiatrist
Dr. Saheli attended medical school at TUMS and completed her post graduate trainings at Rutgers New Jersey Medical School and New York Medical College, where she served as a delegate at the Committee of Interns and Residents. She started her postgraduate journey in Internal Medicine, where counseling anxious patients and effective patient centered individualized communication turned out to be the most rewarding part of her training. This experience led her to pursue integrative wellness approaches to improve patients’ physical and mental health. Dr. Saheli is committed to providing thoughtful and individualized treatment to those she serves. Her hobbies include hiking, dancing, and volunteering in fundraising events for charities and schools.
Aviva Bernat, M.D. – Consulting Physician
Dr. Aviva Shira Bernat is an Integrative and Holistic physician. She completed her medical training in Internal Medicine with sub-specializations in Pain Medicine, Palliative Care and Integrative and Holistic Medicine. She holds board certifications in all three medical specialties. Dr. Bernat brings expertise in fully integrating mental health, physical health, brain health, nutritional medicine and Energy Medicine. She is passionate about bridging these worlds to help her patients heal their bodies, hearts and spirits. She approaches each patient with compassion, humility, patience, commitment and hope.
Tanya Smit, FNP – Nurse Practitioner
Tanya spent 16 years as a nurse in medical-surgery and ICU care before becoming a Family Nurse Practitioner, where she specialized in primary care, urgent, addiction medicine, and medical aesthetics. After caring for these vulnerable populations, Tanya recognized that she wanted to shift trajectories into a mental health-oriented career. Now a psychiatric mental health nurse practitioner student, Tanya’s experience and confidence in ketamine infusions and TMS therapy is unmatched.
Glory Forrest, RN – Infusion Nurse
Glory is a Registered Nurse who is passionate about mental health and wellness and strives to treat her patients holistically. She obtained her Bachelor of Science degree from the UCLA School of Nursing and brings skills and experience from a variety of clinical settings including ER, Medical Surgical Telemetry, Preoperative and Post Anesthesia Care Unit. She is compassionate, empathetic, and enjoys assisting patients through their ketamine infusions.
Janice McKenzie, RN – Infusion Nurse
Janice Mckenzie is a registered nurse who specializes in infusion therapy. She worked for 10 years at a Children’s Hospital providing children with infusions from chemotherapy, biotherapy to antibiotic therapy. Concurrently, she provided children and adults with home infusion therapy as an alternative to hospital admission. She completed her Associates of Nursing degree at College of the Canyons in Valencia, CA and then her advanced degree in Nursing at Walden University in Minneapolis, Minnesota. She holds a national board certification in infusion therapy from the Infusion Nurses Society.
Julia Baschiera, M.A. – Program Coordinator
Julia received a master’s degree in Integrative Health and holds certifications in Holistic Health & Wellness Coaching, Guided Imagery, Patient Navigation and Advocacy and has completed numerous programs pertaining to mindful education. She is currently studying to become an Addiction Recovery & Psychedelic Integration Coach. Julia believes in the power of effectively holding space for others so that they can tap into their natural ability to learn, heal, and grow.
Dominic Marquez-Necochea, BA – Neuromodulation Technician
Ever since he can recall, Dominic has been fascinated with peak performance. He attended San Diego Christian College on a baseball scholarship with dreams of going pro, but was forced to find a different path after a career-ending injury. Dominic recognized that he could take his passion for peak performance and apply it to brain-based treatments. Dominic has expertise and certifications in many neuromodulation modalities, including biofeedback, neurofeedback, and TMS. Dominic brings a unique blend of stability, compassion, friendliness, and maturity to each session.
Andres Cohen, BS – Neuromodulation Technician
Andres graduated from the University of California, San Diego where he helped conduct research focused on understanding how behavioral goals influenced perception, decision making, and memory and understanding the neurobehavioral underpinnings of high-risk behaviors. He brings this passion for research along with TMS experience to the team and believes in the importance of helping patients feel comfortable and well informed with their treatment.
Rayana Villalpando, M.A., PsyD Candidate – Neuromodulation Technician
With over four years in counseling and another four years in TMS, Rayana is our main technician at our downtown office. Rayana is in the process of obtaining her doctorate in Clinical Psychology at Alliant International University. An athlete all her life, Rayana attended Indiana Tech on a full scholarship, and wishes to have a career in helping professional athletes through the retirement process and reducing the stigma of mental health for minority cultures. Rayana has obtained advanced training in CBT, ACT, and psychotherapy.
Mara Farrand, MSc – Patient Coordinator
Since she was young, Mara has been passionate about learning how the brain functions, especially in relation to mental health. Mara attended graduate school at King’s College London and earned a Masters of Science degree in Forensic Mental Health Research. She is excited to use her knowledge of evidence-based treatments for mental health and see the benefits firsthand in a clinical setting. Taking from her experiences abroad, she also hopes to help make mental health care more accessible in America. Mara is delighted to be a part of the team and to be there to guide patients through every step of the process.
Douglas Caine, Psy.D. – Therapist
Dr. Caine is a San Francisco native, musician, recovering addict, and clinical psychologist. He pursued a career in psychotherapy after seeking his own treatment two decades ago, whereby he was astounded at the degree in which he was able to turn his life around. Since then, he obtained his bachelorette and doctoral degrees from Ryokan College, founded a multi-state sober coaching business, and even became a featured provider in A&E’s series Relapse: The Sober Coach Chronicles. Dr. Caine’s dedication, expertise, and unique life experience has earned him an outstanding reputation in depression and anxiety treatment.
Shelby Alkire, LCSW – Therapist
Shelby Chantilly Alkire joins TMS & Brain Health as a Licensed Clinical Social Worker and clinical therapist. Shelby is a graduate of Columbia University School of Social Work and has been practicing in the field of Social Work and Mental Health for over 10 years. Shelby has held many positions and titles within the field of mental health and addiction, including Mental Health Therapist, Family Therapist, Clinical Director, Executive Director, Program Director, and CEO. She also served as an Adjunct Professor in the Social Work department at Azusa Pacific University where she is an alum. Shelby is passionate about helping people integrate their mind and body to experience their emotions holistically. She is trained in EMDR therapy, a modality that treats a wide array of diagnoses including, but not limited to: Post Traumatic Stress Disorder, Generalized Anxiety Disorder, Depression, Panic Disorder, and Chronic Pain. Shelby is also a Registered Yoga Instructor (RYT-200) and has taught yoga classes in San Diego, CA and Indonesia.
Hank Spielberg, Chief Comfort Officer
Hank prides himself in providing additional therapeutic relief to our existing treatments. Hank enjoys greeting new patients at the door, lying next to them during TMS, and subsequently walking them back to the door. When he is not working, Hank enjoys long naps and cheese. Note that due to allergies, Hank only comes into the office by request.
